Physician assessment and Ivermectin orders
We can arrange a health assessment with a physician and talk through Ivermectin as an option. Every order is reviewed by the physician before anything is prescribed.
- Fill out the order form online (secure) or on paper.
- The physician reviews it. We'll call you if anything needs a closer look.
- Pay and pick up or ship. Choose a text or email payment link, or a phone call. Cash or PIN debit at pickup has no card fee.
Using the paper form? Download it, fill it out, and bring it in or call us for the best way to send it.
Online: takes about 5 minutes on a phone or computer, and you sign on the screen. Your information is sent securely to the pharmacy.
Paper: the same form as always. Download the PDF.
Card payments include a 3% card fee. No fee when you pay cash or debit (with PIN) at pickup.
